New Customers
If you are a new customer and want prescriptions filled at Seminole Hartzog Pharmacy, our staff will have you complete a simple patient information form before we can fill any prescriptions.
You will need to bring:
Your insurance card that has the RX billing information included
Valid physical prescriptions OR have your doctor e-scribe or fax your prescriptions directly to us
If you want to transfer existing prescriptions to Seminole Hartzog Pharmacy:
Bring a list of the medications you want transferred to us, or bring the empty bottles from your previous pharmacy.
Contact information for the pharmacy where your prescriptions are currently located.
Your insurance card that has the RX billing information included.
Don’t Forget Your Pets
Seminole Hartzog Pharmacy provides medications for your pets as well as the human members of your family. From dogs and cats to pigs, we help keep plenty of four-legged patients happy and healthy!
Valid prescriptions from a veterinarian are required.
Automatic Refills, Medication Bubble Packing, and More
Our staff knows that life is complicated.That’s why we offer a variety of services to help make managing your medications a little easier.
Automatic Refills
Seminole Hartzog Pharmacy offers an automatic refill service for the majority of your regular medications. Customers who are interested in the automatic refill service are welcome to speak to a member of our pharmacy staff to get started!
Medicine Bubble-Packing
Medications can be confusing, especially for individuals who might need a little help in their daily life. Seminole Hartzog Pharmacy offers a monthly bubble-packing, or pill-packing, service for a small fee.
What is Bubble-Packing?
Bubble-packing is similar to organizing your medications in weekly pill planners, such as the ones that have the days of the week on the lids of the compartments.
Each of your prescription medications will be placed into individual “bubbles” in a “sheet” of 28 compartments.
These bubbles are color-coded and labeled with the name of the medications as well as the date and time that the medications are supposed to be taken.
The number of bubble-packs a patient receives per month will depend on the number of medications and the frequency or time of day that the medications are meant to be taken.

Refill Requests Via App
To request refills via app, please download the RefillQuick app on your mobile device. The RefillQuick system is also accessible via the internet at Refill Quick.
Scan the QR code if you would like to visit the RefillQuick website. You may also click on the QR code to be taken to the RefillQuick website.
To request refills:
Enter (229) 524 - 2223 to find our pharmacy, then you will be redirected to our refill request page.
Input your name and prescription numbers, which can be found on the label of your prescription bottles in the upper left-hand corner.
Please note that if you do not have any refills left on the requested prescriptions we will send a request to your doctor’s office. However, we strongly suggest that you also ask for a refill directly from their office as well.
Insurance
Seminole Hartzog Pharmacy accepts most major insurances. To see if we accept your insurance, please visit your insurance provider’s website for a list of preferred pharmacies.
Seminole Hartzog Pharmacy does not currently accept TRICARE.

Seminole Hartzog Pharmacy does not accept discount cards for medications unless it is a manufacturer’s coupon. Restrictions may apply. Please see manufacturer’s coupon for terms and conditions.
No prescription drug insurance?
We can help!
At Seminole Hartzog Pharmacy, we understand that having prescription drug insurance isn’t always possible.
Our pharmacy staff works hard to be able to offer low cash prices on the majority of generic medications. Sometimes we even beat insurance copays!
Over-the-Counter Medications

Seminole Hartzog Pharmacy works with our wholesale partners to offer a wide variety of brand name, hard-to-find, and generic OTC, or over-the-counter, medications. Including the cough syrup from your childhood that you can still taste whenever you smell artificial cherries.
